The care journey

Assessment

An RN first understands current needs, medical instructions, the home setting, and the family’s concerns.

Care Plan

Priorities, care scope, and what needs to be prepared are put into a workable plan.

Coordination

Nurses, relevant professionals, supplies, or the existing care team are coordinated where needed.

Clinical Care

Appropriate clinical home nursing is delivered in line with medical instructions and professional processes.

Review

When circumstances change, the plan is reviewed so the family knows what to watch for.

When should urgent medical care be sought?

For acute breathing difficulty, reduced consciousness, heavy bleeding, severe pain, or another emergency, call 999 or go to an emergency department immediately. Home care does not replace emergency care.
